Cranium Rats are a type of rat beast found in the Undercity Ruins and its surrounding areas in Act Three. The Netherbrain speaks through the rats in unison.

Cranium Rats are temporarily hostile when wandering too close to the Morphic Pool Dock before obtaining all three Netherstones.
Attacking a Cranium Rat will engage their passive feature Infectious Madness, possibly causing in the attacker. This passive will not trigger when attacking with area spells like .
