Abdirak resides inside the Shattered Sanctum next to the torture room. He offers peace and serenity through pain. If accepting, Abdirak strikes the player character several times. If he is pleased with their performance, he blesses them with the permanent bonus , and the following inspiration:

The player character must endure three strikes and pass each performance roll three times, each intimidation roll three times, or fail each constitution roll three times. If attempting to fail the Constitution rolls, the player character can self-inflict

- If reading On Receiving Her Grace, found on the nearby table, then the party can unlock the dialogue option "I saw your book. Let me try some of those techniques". This allows them to choose which tool – mace, knife, or axe – Abdirak uses, and thus changes the damage type inflicted to , , or respectively. This can be used to reduce the damage taken. For example, selecting the axe option and casting or equipping the Amulet of the Unworthy[1] to gain Slashing resistance. If all three ritual weapons are taken away, he casts instead.
- Abdirak leaves the sanctum if the goblin leaders are killed. He can be found during the fight standing in front of a burrow contemplating that he is too big to fit to escape. He is carrying the Ritual Dagger and Ritual Axe on him from the Bloodied Shrine.
- After leaving the Shattered Sanctum Abdirak, unlike most other fleeing characters, does not despawn for a very long time. It is possible to follow him all the way out of the goblin camp, through the bridge, until only a bit south from Blighted Village where dirt road turns to stone, where he finally despawns.
